Request for Assistance and Contact Details

What is Request for Assistance (RFA)?

Request for Assistance is the new system which has replaced our previous referral procedure. We have an open RFA system which means parents/carers as well as professionals, can complete and return a request for assistance form. Alternatively anyone can contact the department to discuss any potential request to Occupational Therapy.

If a Request for Assistance is appropriate then Occupational Therapy input may include one or more of the following:
  • Advice
  • Strategies
  • Information Sessions
  • Assessment
  • Direct Input
  • Joint working with others involved with your child
