Photographic and Video Consent

You will be asked to consent to having your photographs/video taken at the hospital. You have the right to know how your photographs/videos are used and you have the right to change your mind about the permission you have agreed to at any time. If you wish to withdraw your consent please contact the Department as soon as possible after your attendance. Please note that it will not be possible to remove photographs or videos from the public domain once they have been published.

Different Levels of Consent

When signing the photography patient consent form, there are 4 different levels of consent and it is entirely your decision to choose which level you would like to sign for:

Level 1 - Medical records: You give your permission for clinical images or video recordings to be taken for the purpose of medical records only. These confidential images or videos will remain within the hospital.
Level 2 - Teaching: You give your permission to the above (1) and for clinical images or video recordings to be taken for teaching purposes. These images or videos may be used for teaching other healthcare professionals, both within the hospital and at external medical conferences and meetings.
Level 3 - Patient to patient: You give your permission to the above (1 & 2) and for clinical images or video recordings to be shown to other patients who may be considering the same course of treatment. These confidential images or videos will remain within the hospital.
Level 4 - Publication: You give your permission to the above (1,2 & 3) and for clinical images or video recordings to be taken for publication purposes. These images or videos can then be used both inside and outside of the hospital such as in medical journals, medical conferences, medical textbooks and or medical websites. This is publication in principle; you will be contacted again for specific consent prior to the release of any clinical image or video for publication purposes.  If you decide to withdraw your consent at a later date - it may not be possible to recover them, once in the public domain. Images in the public domain may also be difficult to regulate.
