Dundee Health and Social Care Partnership

In April 2016 parts of NHS Tayside and Dundee City Council joined together to deliver Health and Social Care Services to people in Dundee by forming the Dundee Health and Social Care Partnership. The staff within the Partnership deliver Integrated Health and Social Care services and supports.

What do we do?

Integrated Health and Social Care Services help to keep individuals and the communities they live in healthy and well for as long as possible. Staff from NHS Tayside and Dundee City Council work with others to improve health and well-being and reduce inequalities, to support people through periods of personal and family crisis, and to provide the joined up and seamless support people need to live safely and independently in their own homes (or a homely setting) for as long as possible.

We work very closely with our partners in the Voluntary Sector, people in local Communities and other providers of care in the City. Along with all of our partners we are committed to ensuring that each citizen of Dundee will have access to the information and support that they need to live a fulfilled life.

Our Services

The Dundee Health and Social Care Partnership are responsible for all community based health and social care services for adults in Dundee including:
  • Social Work and Social Care Services for Adults and Older People
  • Adult Mental Health Services
  • Older Peoples Health services
  • District nursing services
  • Long Term Conditions Support
  • Learning Disability Services
  • Health Inequality focussed Services (Dundee Healthy Living Initiative/Equally Well/SoS/Keep Well)
  • Occupational Therapy and Equipment Services
  • Protecting People Services
  • Carers Support

We are also responsible for some Tayside-wide health services including Specialist Palliative Care, Sexual and Reproductive Health Services, Psychology Services, Dietetics and The Centre for Brain Injury Rehabilitation.

Who Are We?

Although Integrated Health and Social Care mean some changes, the staff group will remain the same. Through integration staff will be supported to work more effectively and you will have the best possible experience of services and support.
