Mrs C complained to me about the care and treatment that her mother (Mrs A) received from Tayside NHS Board (the Board). In May 2017, Mrs A was diagnosed with renal cell carcinoma (a type of kidney cancer) and she was referred for kidney surgery to treat it. Following her kidney surgery in August 2017, Mrs A developed excess fluid around her lungs and an infection; and her condition continued to worsen. In late September 2017, Mrs A was discharged home for end of life care and she died the next day.

Mrs C, an advocacy worker, complained to me, on behalf of Ms A, about the care and treatment that Tayside NHS Board (the Board) provided to Ms A. From early 2012 onwards, Ms A experienced severe hip pain following her right hip replacement surgery. It affected her ability to walk and to carry out everyday tasks. Despite various orthopaedic reviews and investigations over the following five years, no underlying cause was identified for her pain. In mid-2017, Ms A's symptoms suddenly worsened and she experienced total right hip replacement failure. Ms A was referred for further surgery and a deep-seated infection was found in her right hip joint. Mrs C complained about an unreasonable delay in diagnosing Ms A's hip infection.

Miss C complained about the care and treatment her late brother (Mr A) received from Tayside NHS Board (the Board). Mr A had type 1 diabetes with recurrent episodes of hypoglycaemia (when the level of sugar (glucose) in the blood falls below a set point) and a learning disability. Mr A, who had been a patient with the Board’s diabetes service since he was a teenager, died unexpectedly aged 38 years.

Summary - Mr C complained to us that the board had failed to provide his wife (Ms C) with appropriate clinical treatment following a GP referral to Perth Royal Infirmary for a suspected brain aneurysm. Ms C had been referred to the hospital by a GP after becoming unwell. In the referral letter, the GP referred to, amongst other things, a suspected subarachnoid haemorrhage (an uncommon type of stroke caused by bleeding on the surface of the brain). Ms C had reported sudden onset of pain in her head and neck with some visual disturbance. She was admitted directly to the acute medical unit in the hospital where she was medically assessed by a specialist trainee doctor. She was then reviewed by a consultant physician. She was subsequently discharged home with the problem felt to be musculoskeletal.

Overview -The complainant (Mr C) raised a number of concerns that his mother in law (Mrs A) had been inappropriately cared for in Perth Royal Infirmary. Mrs A had been admitted on 15 February 2013, with a sudden loss of mobility. She was discharged on 13 May 2013, but had not regained the ability to walk. Mr C said that it was not until later that the family learned Mrs A had suffered a fractured hip. Mr C said this was not properly diagnosed or treated and that Mrs A was never x-rayed during her stay in hospital. Mr C was also unhappy with the way his complaints were handled by Tayside NHS Board (the Board), as he felt the internal review process lacked objectivity and dismissed the family's concerns.
