Nursing and Public Health
We all have a responsibility for our own health. The Community Health Partnership also have a pivotal role to play in raising health awareness, reducing health inequalities, promoting healthy lifestyles and delivering community based initiatives in consultation with you, the public.
The Community Health Partnership works with other organisations such as Perth & Kinross Council and local voluntary agencies to improve the health of the Perth & Kinross population and to address the key challenges to achieving better health such as reducing levels of smoking, increasing physical activity, improving diet and reducing levels of alcohol consumption.

Community Nursing Services (District Nursing)
The Community Nursing Service provides nursing care to patients in their
own home or residential care setting. The Community Nursing Team will
carry out an assessment of both health and social care needs, as well
as an assessment and arrangement of any equipment that the patient may
need.
The team will provide ongoing assessment, management and implementation
of any nursing care required in the home.
They will also provide support and advice to patients and carers to maximise
independence, as well as advice with regard to health promotion.
Community Nursing Teams have bases throughout Perth & Kinross. To

Community Public Health Nursing (Health Visitors)
Health Visitors and Community Public Health Nurses are health professionals
who provide support and access to services in line with specific patient
needs. They work in partnership with clients and have links to GPs, Social
Work Services and Housing, Education, Community Hospitals and Voluntary
Organisations. They deliver direct care, and provide confidential advice
and support.
Some examples of the services offered are thing such as Health Advice
drop in sessions, Post Natal Support Groups and Smoking Cessation Sessions.

Specialist Palliative Services
The Palliative Care Team works closely with other health and social service
professionals across the community and hospital settings. The team includes
Community Macmillan Nurses, Hospital Macmillan Nurses, and at Macmillan
House a full range of health professionals including complementary therapists
and volunteers.
The team focuses on achieving the best possible quality of life for patients
and families facing the problems associated with life-threatening illness.
The aim is always to maximise physical, psychological, social and spiritual
well-being, and enable personal choice, through skilled assessment and
treatment planning. Patients are supported in their own homes, whilst
in hospital, or by attending Macmillan House Day Services.

Central Healthcare
Central Healthcare provides a nurse led service for those who find themselves
homeless or vulnerably housed.
The service offers both a specialised outreach service which includes the young peoples health team as well as a GP practice and maintains strong links with mainstream services.
The Central Healthcare Team provides specialist care to the following
client groups:
- Those with Mental Health Problems
- Those with Drug & Alcohol Problems
- Gypsy Travellers
- Homeless Families
- Children in Looked After Care
- Young People
- Single Persons
- Local Authority Care Leavers

Learning Disabilities Nursing
With strong links to the specialist Tayside service, Learning Disabilities
Nurses work jointly with Social Work colleagues to provide a comprehensive
service for adults with learning disabilities.
Within Perth & Kinross, there are Community Learning Disability Nursing
Services, Community Day Services based in Scone, and there are also staff
within two community houses in Perth.

Family Planning Services & Sexual Health
