Perth & Kinross Partnership Services For Older People

Introduction:

Perth & Kinross (P&K) Partnership Services for Older People deliver joint health and social care services to those patients over the age of 65 in Perth & Kinross.

The aims of the services are based on the four national outcomes for older people:

  • Support more people at home, as an alternative to residential and nursing care.
  • Assist people to lead independent lives through reducing inappropriate admissions to hospital, reducing time spent inappropriately in hospital, and enabling supported and faster discharge from hospital.
  • Ensuring people receive an improved quality of care through faster access to services, and better quality services
  • Better involvement and support of carers.

We understand the importance of involving the users of our services in the planning, developing and commissioning of new or existing services, and consultation with service users is a key part of our joint planning structure.

Medicine for the Elderly wards at Perth Royal Infirmary

There are three rehabilitation wards within Medicine for the Elderly, based at Perth Royal Infirmary.

Tay and Earn Wards provide rehabilitative care to patients over the age of 65, from a multidisciplinary team, experienced in Elderly Medicine, Orthopaedic Rehabilitation, Surgical Rehabilitation and Stroke Care.

The Stroke Unit provides a specialist, co-ordinated approach to the rehabilitation of stroke patients. The care is delivered by a multi-disciplinary stroke team who all have specialist skills and knowledge in the area of stroke rehabilitation. The stroke team helps to set realistic goals for patients throughout their stay in the unit. To do this, the team meets once a week to discuss the care and progress of each patient.

Psychiatry of Old Age and Dementia

Psychiatry of Old Age Services work closely with Health & Social Care Service professionals across the Murray Royal and community hospital settings.

The service focuses on achieving the best quality of life for patients over the age of 65 and their families facing the problems associated with mental health. The aim is to maximise psychological and physical wellbeing and enable personal choice (where appropriate) through skilled assessment and treatment planning.

To access psychiatry of old age services, a referral through single point of entry process will enable an appointment with a specialised professional who will assess your present needs and involve you in the planning of your care.

The services provided include:

Inpatient and day facilities at Murray Royal Hospital

There are four inpatient wards, and these are Tummel, Garry, Kinclaven and Leven Ward. The two day centres are Kinclaven Day Centre and Kinfauns Day Centre.

Older people’s community mental health teams

There are four Older Peoples Community Mental Health Teams in Perth & Kinross, and these cover the localities of Perth City, Northwest Perthshire, Southwest Perthshire and Strathmore.

The teams endeavour to work together to help people with mental health problems including:-

  • People of all ages with cognitive deficits, which may lead to diagnosis of dementia, and people already diagnosed who need further support/assessment.
  • People over the age of 65 years with functional mental health problems.
  • The team also work with relatives and carers offering advice, information and support.
  • The teams will assess and review needs to provide a range of treatments and arrange services.
  • All teams have input from Occupational Therapist, Technical Instructors, Physiotherapist, and Social Work department staff.

All referrals come into one point of entry, addressed to the OPCMHT ( Perth City, Strathmore, SWP, NWP Locality) Murray Royal Hospital.

  • Routine referrals will be discussed and allocated on first working day of each week.
  • Urgent referrals should be clearly marked as such, and a senior member of staff shall contact you to discuss.

Telephone referrals will be accepted on the understanding that they will be followed up by clinical request letter.

Community Hospitals

Community hospitals within Perth & Kinross offer holistic care to patients who do not require the full range of diagnostic services that are normally provided within an acute hospital.

Patients within Community Hospitals have their continuing health care needs delivered by doctors from the local GP practice/s, as well as dedicated nursing, Occupational Therapy and Physiotherapy staff and other Allied Health Professionals. Within the Community Hospitals there are also day hospitals providing assessment and rehabilitation services.

Care Homes

Community Care provides a whole range of services which aim to support the residents of Perth & Kinross when moving onto a residential or nursing home setting.
Included in this is Beechgrove House and Parkdale which are both council owned and provide residential based care for the elderly in the area.

Financial Information with regards to entering Residential or Nursing Care is also available, provided by the Community Care Finance Team.

Scottish Care in partnership with private and voluntary care home representatives have developed a website which covers all Scottish Care linked homes in the area.

Home Care

The aim of home care is to provide personal, social and domestic care to enable people to live independent lives in the community for as long as possible.

Home Care is available to all vulnerable adults; families with children experiencing social/health problems; and as a means of supporting carers. Services appropriate to the need of the individual will be provided on the basis of the assessment, carried out by social work teams, subject to the eligibility criteria.

Social Work Teams

The aim of the social work teams across Perth & Kinross is to deliver services to adults and older people who are vulnerable due to age, frailty or disability based on an assessment of their needs. The principles underlying assessment and intervention are:

  • Respecting the right of self determination
  • Promoting participation
  • Taking a whole person approach
  • Understanding each individual in the context of family and community
  • Identifying and building on strengths

Assessment of needs will be carried out, and appropriate services co-ordinated, monitored and reviewed to ensure that changing needs are identified and addressed, taking into account eligibility criteria and available resources. Welfare benefit checks will be undertaken to ensure income maximisation.

Intermediate Care Services (definition)

Day Services

Day Services for adults over 65 years of age provide support to enable individuals to live as independently as possible within their own homes, thus preventing the need for admission to long term care.

Patients are encouraged to participate in chosen activities taking into account their individual need. A balance of physical and mental stimulation is on offer as well as small group activities to meet specific needs.
Subject to specific criteria, some day services may offer a service to patients with dementia or memory problems. This may include short term support following hospital admission, support following bereavement, carer relief, concerns of risk to safety or self neglect.

Intensive Care at Home Service (ICAHS)

ICAHS is a needs led service which is a joint venture between NHS Tayside and Social Work Services, and has been running for approximately 5 years.
Patients using this service can do so for a maximum of 10 days, and during this time the patients needs are continually assessed and reviewed.

The aim of the service is to support people in their own homes in either an assisting or enabling role whilst allowing either Early Supported Discharge from hospital or eliminating the need for admittance into hospital.

Community Alarm

The Community Alarm Service is an assistive service in that it contributes to an existing care package thus allowing clients to remain in their own homes longer. It enables patients to feel safe in their own homes, in the knowledge that should they require assistance they can activate their alarm and it will be delivered either by community alarm contacting the named key holders or responding themselves.

Community Alarm provides a 24 hour control and response service to just over 4,000 people in Perth & Kinross. The service covers people in their own homes or in sheltered housing. Community alarm responds to all sheltered housing clients when sheltered housing wardens are not on duty.
