Community Nursing Services (District Nurse Angus)

The Community Nursing Service provides care to people in their own home or home care setting. The District Nursing Team will assess the health care needs of patients and families, monitor the quality of care they receive and be professionally accountable to care delivery.


As well as providing direct care to patients the team have a teaching role, working with patients to enable them to care for themselves or with family and carers teaching them to provide care to their relatives.

The District Nursing Team also has a role in preventing admission to hospital or re-admission to hospitals allowing patients to be cared for at home in partnership with other services and carers.
