Crisis Resolution and Home Treatment Team, based at Carseview Centre.
For the purpose of this referral guide, this service is intended for Emergency presentations that require review within 4 hours.
The least restrictive option of support should be considered which may include DBI, PALMS, Third sector, Community support, Crisis centres (such as Hope Point in Dundee, The Beacon in Angus and the Neuk in Perth & Kinross)
Please also note that urgent (within 72 hours) and routine assessment requests have separate referral pathways.
General Practice assessment can be in the form of face to face assessment or remote consultation providing the assessment is adequate and covers all the information required as seen in the ‘Primary Care referral information’ section. The referral may be declined by CRHTT following discussion with the referrer if not appropriate for the service.

Who to refer
• Emergency Mental Health Referrals – need to be reviewed within 4 hours
• Are considered to be a significant risk to themselves and/or others
• May require admission or Intensive Home Treatment
• Must be assessed to be medically fit

Who not to refer
• Any patient who does not need an Emergency assessment of their mental health within 4 hours

Angus GP Practices:
Perth and Kinross GP Practices:
Dundee GP Practices:
- Angus locality CMHT’s manage all emergency mental health referrals in-hours (0900-1600hrs).
- Phone the CRHTT after 1600hrs on 01382 423053
Perth and Kinross GP Practices:
- ‘New’ emergency mental health referrals (ie those patients not already open to the CMHT) refer to the CRHTT.
- Emergency mental health referrals for those patients already open to P&K CMHT’s should be directed to the CMHT Duty Worker in the respective P&K locality (North, South, City)
- North and South CMHTs 0900-1500hrs,
- Perth City CMHT 0900-1530hrs.

Primary Care referral information
Please supply the following information when referral is being made:
- Basic demographic information – a contact number for the patient essential
- Presenting problem
- Existing mental health diagnosis
- Open to mental health services
- Past psychiatric history
- Physical Health issues/ Disability
- Involvement of other agencies
- Social setting/ circumstances/ child protection
- Next of Kin details and contact number
- Drug and Alcohol use
- Medication/ Known allergies
- Risk (Suicide, Self-Harm, Neglect, Risk to Children, Risk to Others, Known Forensic History, Violence & Aggression)
- Transport arrangements
- Disengagement plan (actions to be taken in the event the patient fails to attend the specified appointment)
