Community Nursing Service

The Community Nursing Service is staffed by teams of qualified nurses, health care assistant/nursing auxiliaries, each with an important role to play.
 

WE PROVIDE A 24HOUR ON DUTY SERVICE.
 

DISTRICT NURSING SISTER is a first level nurse with additional training and practising at an advanced level. She is responsible for carrying a caseload of patients. Many are also Team Leaders, who, as well as being responsible for managing her caseload, prioritises and delegates appropriately to other members of the team. She is also responsible for motivating and developing knowledge in the team by determining specific abilities and interests of team members through personal and professional developement plans.
 

Team Leaders are also committed to undertake further training to expand their knowledge in research and developments which they can then impart to their team thus ensuring optimum knowledge based patient care.
 

As well as looking after patients, carers and team members, District Nursing Sisters are also responsible for working closely with GP's, and other members of the Primary Care Team, Social Work Department, Statutory and voluntary bodies, hospital staff and their Team Manager.
 

COMMUNITY STAFF NURSES are first level nurses who may have an additional District Nursing qualification and who work under the direction of the Team Leader.
 

HEALTH CARE ASSISTANT/NURSING AUXILIARY is trained in basic nursing skills. With the agreement of the Team Leader, they can be given further training to develop their role.

Accessing the Community Nursing Service

Referrals mainly come from G.P., Hospital, Social Work Department, Family and Friends although an open referral system is in place.

Anyone wishing the District Nurse to visit can do so by telephoning the patient's GP within surgery hours.
Outwith these hours the service can be accessed by calling the NHS 24hour number.
