Who We Are

• 50 minute confidential appointments for talking about grief/loss, challenging relationships, stress, work, illness, caring responsibilities etc
• Active listening, assets-based model
• Patient is in control of subject matter – whatever they find challenging at the time
• Flexible timing of follow up appts. (usually up to 6 max.)
• Appointments usually available within a week (9-5 mon-fri) by phone or F2F in GP practice

Who to Refer

• Age 16+ and registered with an NHS Tayside GP practice
• Struggling emotionally with any challenging life situation: health (theirs or family), relationships (work or home), loss/bereavement, life changes etc.
• Community or outpatient (for inpatient support, contact chaplain 01382 423 110)

Who Not to Refer

• those requiring a long-term befriender or practical support (see social prescribing services instead)
• anyone actively under the influence of alcohol/drugs and requiring specialist addiction services
• those with severe or enduring MH diagnosis who require specialist MH input
• anyone actively in crisis

Alternatives to Referral

If unsure if right referral, we can speak to patient and cross-refer to social prescribing, third sector services etc or back to GP if needed.
