About Us
There are 95 care homes in Tayside which are mostly for older people, and most of these are privately owned. The Tayside Care Home Collaborative Board and Collaborative Care Home Support Teams (CCHSTs) are committed to the development and implementation of effective, collaborative care across all care homes in each of the Health and Social Care Partnership located in Angus, Dundee, and Perth & Kinross (P&K).
The CCHSTs work in partnership with the wider Multi-disciplinary Teams, Care Home staff and providers to address the recommendations set out in the My Health, My Care, My Home healthcare framework for adults living in care homes in Scotland. The recommendations, developed by key stakeholders including care home residents and their families aim to ensure to ensure everyone living in a care home experiences the best possible care.
The healthcare framework is guided by the principles of realistic medicine which focus on what we need to live well and what matters most to the person receiving care. 
By building relationships between our teams we have created a culture of collaborative decision making, shared learning and assessments of needs which has strengthened person-centered interventions and collaborative relationships between our teams and care homes.
