CONNECT (Supporting Early Intervention in Psychosis)
Service Description
CONNECT aims to provide a range of early interventions for people aged between 16 and 35 years old who are experiencing their first episode of psychosis. The team of mental health nurses, clinical psychologists, psychiatrists, peer support workers and healthcare support workers will offer specialist assessment, care and therapeutic input to help improve individuals’ long-term recovery prospects.
Who to Refer
Criteria
- Aged between 16 and 35 years old
- Currently living in the Dundee area
- Experiencing symptoms of psychosis (hallucinations, delusions, disordered thinking) for the first time, or there has been a previous experience of psychosis but no prior active treatment
- Symptoms have lasted for longer than one week, and are sustained or experienced multiple times during the day
- These experiences are causing significant distress and/or deterioration in ability to function and carry out your normal routines.
How to Refer
- Anyone can refer to the service as long as the person being referred has provided consent and the referrer is able to answer the questions for screening and referral.
- Complete the referral criteria screening questions with the person (below).
- Telephone the service 01382 835514 ext. 59075 and a clinician within the team can triage and take the referral over the phone.
- If a clinician is not available at the time of your call you can complete our referral and screening form and email to us at – Tay.dundeeeipservice@nhs.scot. A clinician will review the forms and advise of outcome within 24 working hours. Please ensure contact details for yourself and the person being referred are clear on referral form. If you do not receive an automated delivery receipt phone us on 01382 835514 to ensure we have received the referral.
