- Angus Falls Reduction & Safer Mobility Service can accept referrals for people who have experienced falls in the community.
- Angus safer mobility team aim to make first contact with a person within 2 working days of receipt of referral, when we will complete a level 1 falls assessment screening.
- Level 1 screening may include a telephone call with the person that will include a review of their general health along with a falls history.
- Where level 1 screening identifies further assessment is required, the safer mobility team aim to complete a level 2 multi factorial falls assessment within a person’s home within 15 working days.
- A level 2 assessment considers others factors that may increase the risk of falls. For example; vision, hearing, foot health and foot wear, home environmental risk factors, continence, balance and gait, social factors, medication compliance, walking aids.
- Level 2 falls assessment may result in targeted referral to; other health care professionals (Physiotherapy, Occupational therapy, Podiatry, General Practitioner, Optician, Audiology), Social Services, better balance class, third sector charities, Angus Council keep active activity services (Angus Alive) and voluntary services (Angus voluntary action) (health walks).

Who to Refer
- People with a history of falls where the underlying cause cannot readily be established
- People with a fear of falling
- People who feel unsteady or have poor balance
- People who had dizziness/blackouts/unconsciousness at the time of fall
- People with any of the above and a medical history of osteoporosis (or family history of same)
The aim of referral is early intervention to assess current falls history and reduce risks of further falls, promoting regular activity and reducing potential for de-conditioning.
Who not to Refer
- People who have engaged with Angus safer mobility & Falls services within the previous 12 weeks
- People who are currently referred to or attending community Physiotherapy
- People who have a history of frequent falls (due to a medical condition) and have had input from community rehabilitation team (CRT) where mobility and assisted living resources are already in place
- Please do not refer as a ‘catch all service’ this can result in delay referring to appropriate service(s)
