Specialty Description

Sources of Support is a social prescribing service within primary care which operates in all GP Practices in Dundee ONLY. Our team consists of Primary Care Link Workers and Primary Care Associate Practitioners (support workers). Social prescribing services can differ in their approach and remit. In Dundee, our remit is to work with patients whose physical, mental health and wellbeing is impacted by social and economic issues which means that the service will offer non-medical interventions and co-ordinated care to help improve the patients’ health and wellbeing.

How We Work

There are a range of issues that we can support patients to identify non-medical interventions which may include but are not limited to:

• Core needs (food, energy, clothing)
• Housing issues inc. homelessness
• Domestic abuse
• Counselling
• Family relationships
• Daily structure and routines
• Social anxiety
• Social isolation
• Employment, training or volunteering
• Loss of confidence or purpose
• Supported visits to appointments
• Financial issues

The Primary Care Link Workers carry out a patient assessment and case manage their needs for up to 20 weeks, helping them achieve their identified goals. Together we will create a plan of action which will include referrals to other services, and community supports. Primary Care Associate Practitioners may also support patients within the 20-week episode of care. Advocacy and liaison with primary and secondary care, statutory and third-sector services is a key feature of the Primary Care Link Worker role.

Who to refer

The Service will accept adult patients (>16 years) who meet the following criteria:

• Physical, mental health and wellbeing impacted by social, economic or personal issues.
• Patients who have very limited resource in terms of managing services and systems and who would benefit from co-ordinated care for up to 20 weeks.

Who not to refer

Some patients' needs will not meet the criteria of the service. We advise that the patients below are directed to the relevant specialist service:

• Patients with unmanaged / unsupported substance use issues.
• Patients who are experiencing a mental health crisis, including acute episodes of psychosis

How to refer

Referrals will be accepted from GPs or any members of the wider practice team including reception staff within the Dundee area.
