We are a small team consisting of highly experienced physiotherapist and AHP support workers.
Our primary aim is to identify and assess adults over the age of 18 in the community who are new to falling or who have been identified as having increased risk of falling, and have no or little previous input from AHP’s in terms of fall prevention.
We provide a comprehensive multi factorial falls risk assessment, either by telephone or in the individuals own home, with onward referral to the different services that may assist the individual in reducing their falls risk, We also provide physiotherapy assessment and evidence based falls treatment interventions for these individuals. We do not provide long term rehabilitation, and if this is required, will refer onto the appropriate team.

Who to refer
- Anyone over the age of 18
- History of New falls or reduced balance or beginning to lose confidence
Who not to refer
- People who have had previous input from Falls service- should only be re-referred if there has been a definite change in circumstance
