Preparing you and your family for discharge begins as soon as your child enters the hospital. Your named nurse will work with you, your child and the health care team to co-ordinate your childs discharge from hospital.
Your child may require medication/dressings/supplies to take home. It can take a little time to prepare your medications for home but where possible we will try to prepare this in advance. A discharge letter will also be sent to your GP for information.
If you have any questions or concerns during or after your child’s stay in hospital please do not hesitate to contact the ward or your GP.
After a hospital stay your child may require follow-up and an outpatient appointment for this will be sent to you in the post.
