Self-Referral to MSK Outpatients
If you are in Dundee, you can also use the self-referral form below to be added to the waiting list for MSK Outpatients Physiotherapy. Please note this is not an emergency service and should your symptoms worsen while you are waiting to be seen please contact your GP or FCP. You can still be referred to MSK Outpatient service by your GP or FCP instead.

There is also a print version, which can be completed by hand and posted, or scanned and emailed.
