Think First Contact Physiotherapy First

If you have a ‘musculoskeletal’ problem (such as a sore back or sports injury), in the first instance contact your GP to organise a prompt assessment with the First Contact Physiotherapy (FCP) Service or a GP.

Self-Referral

If you are in Dundee, you may use the self-referral form below to be added to the waiting list for MSK Physiotherapy. Please note this is not an emergency service and should your symptoms worsen while you are waiting to be seen please contact your GP or FCP.

Important Information for using these forms

Use the icons below to download the form (on some browsers you will need to right click and save the link to be able to open the form). The editable version can be completed on your computer or phone, then please save and email it to the Physiotherapy team. There is also a print version, which can be completed by hand and posted, or scanned and emailed.

For those in Angus or Perth & Kinross

You can access First Contact Physiotherapy by calling your GP reception. For the Outpatient service you can be referred by your GP or your FCP Clinician.
