Management of conditions
Long term management of conditions - we can help to problem solve what impairment is causing movement or walking difficulties and offer ways to improve or manage this.
Setting goals - We can help you to give yourself realistic goals to achieve and ways to achieve them. For example, this may be a goal to walk to your local shop, or stand at the kitchen counter for longer to achieve more in the kitchen, or improving your arm function to help with getting dressed. We can work with low level goals right through to the higher level performers and their goals.
Self management - We can help to devise strategies to enable you to self manage your long term condition, taking ownership of your symptoms and how to manage them.
Link with other services in the NHS - We can help identify what services may be of benefit to you and your symptoms, and we can complete the referrals to help you get the support you need. This may be occupational therapy for equipment or to work on your cognition, or orthotics to look at splints and braces that may help maintain your independence.
Other resources - We have a broad knowledge of all the other services that are available to you outside of the NHS in your area, this may be exercise classes, social project, support groups and many more options to help you manage your condition.
