Frailty at Home (Physiotherapy)
The Frailty at Home Service features wide ranging team including Geriatricians, Advanced Practice Nurses, Frailty Nurses, Physiotherapists and Occupational Therapists. The service aims to optimise the care of older people living with frailty syndromes closer to home, while supporting early hospital discharge and prevention of admission.
Frailty at Home Physiotherapy provides a rapid response service to acute deterioration in mobility for people living with frailty, with the aim of preventing unnecessary hospital admissions, which can be detrimental to the frail and elderly.
How Can Frailty at Home Physiotherapy help?
Physiotherapy provide a rapid response to acute deterioration in mobility. You may be referred to this service by the Scottish Ambulance Service or via the Frailty nurses if admission to hospital is a risk. Physiotherapy can help by:
- Assessing sudden changes in your mobility.
- Supporting recovery following acute illness.
- Rapid assessment and issuing of walking aids and equipment.
- Assessing strength, balance and walking.
- Supporting you to regain confidence and independence.
- Providing individualised exercise and rehabilitation.
- Helping you develop strategies to remain active and manage at home.
- Streamlining Prevention of Admission care into community rehabilitation services.
We are a short term prevention of admission service, and will step you down to the Dundee Community Rehabilitation Team once early admission risks have been improved.
Who May Be Referred?
Referrals will be made by the Scottish Ambulance Service or via the Frailty at Home medical and nursing staff if there is:
- Sudden deterioration in mobility or ability to transfer.
- “Off legs” presentation.
- Recent and sudden increase in falls.
- Post-infection or post-illness deconditioning risking admission.
- Functional decline following hospital discharge risking readmission.
- Difficulty managing safely at home due to physical decline, risking hospital admission.
