Social Work Support

The First Contact Team is the first point of contact for adults aged 16 and over who live in Dundee and need help, advice or support but do not already have an active Social Work worker or District Nurse.

The team provides a quick referral, assessment and, where possible, access to the most appropriate service.

The First Contact Team works with a range of health and social care services to help people access the support they need.

What Can The First Contact Team Help With

The team can provide information, advice, assessment and referrals for a range of support needs, including:

  • Social care and support at home.
  • Help with everyday activities and maintaining independence.
  • Home care.
  • Occupational Therapy assessments, equipment and adaptations.
  • Community meals.
  • Community Alarm services.
  • Nursing equipment.
  • Housing-related support.
  • Day care services.
  • Emergency respite.
  • Other health and social care services.

    The service is available to individuals, relatives and carers, as well as healthcare and other professionals.
