NHS Tayside now uses a digital pre-assessment application called MyPreOp, provided by Ultramed. All patents will be sent a text with a link to a health questionnaire. It is important to complete this health questionnaire in full as soon as you can. This is a vital step in your journey towards surgery. If you require help to fill in the health questionnaire you could ask a family member to help or you can contact the pre-assessment team on 01382 740743.
Once your health questionnaire is returned it will be reviewed by a member of our pre-assessment team. If the team have any further questions, they may send the questionnaire back to you or telephone you for further information. If you are on any medication you may find it helpful to have your prescription with you as you fill out the questionnaire.
Some patients will need to come to the pre-assessment clinic for a consultation with one of our nurses or to have tests performed. If this applies to you, a member of the pre-assessment team will contact you to arrange this. Occasionally patients may need to attend a clinic with a pre-assessment anaesthetic consultant. The team will let you know if you require this.

If you are started on any new medications after your pre-assessment please call the pre-assessment team to let them know as you may need additional advice for these around the time of your operation.
