Please download and complete the below form and then it can be submitted in three ways:
1. Hand it in to the Respiratory Laboratory by hand (there is a slot for the forms outside the office).
2. Send it via internal mail to: Pulmonary Function Lab, East Block, Level 4, Ninewells Hospital, Dundee, DD1 9SY.
3. Scan and attach it to an e-mail and send to: tay.pulmonaryfunctionteam@nhs.scot.
