- Call Whitefriars once email sent in order to arrange follow up appointment for patient.

New start on Warfarin:
Stable (fragmin discontinued and 2x INR’s in therapeutic range)
- Complete new referral form; available in all wards or from CIU.
- Take new referral form and in-patient anticoagulant sheet to CIU for it to be emailed to Whitefriars
- Refer patient to Whitefriars & call them for an appointment
