If you cannot find the information that you are looking for by searching the NHS Tayside website or through the NHS Tayside Guide to Information then you can make a Request for Information. The following guidance should help you make your request.

What information do we need to process your request?

When you request information from us you will have to provide us with enough information to ensure that we can promptly process your request. Therefore, to avoid delay, you should include the following information:

  • your full name, full address and if relevant, your e-mail address or phone number
  • your preferred method of communication
  • the topic of your request together with a description, focussing on the information you require
  • the date range associated with your request

What will happen to your request?

Any request for information can be sent directly to the Information Governance Offices at NHS Tayside to the following email address:

tay.foisa@nhs.scot

Alternatively, you can send your request by post to the following address:

The Information Governance team will respond to your request by:
  • sending an acknowledgement of your request
  • asking for you to provide further information, if required
  • responding to you within 20 working days
