What to Expect
One of our team members will contact you by telephone to arrange a convenient date and time for your appointment.
Most appointments take place in your home so that we can see how you manage in your usual surroundings.
Occasionally, we may ask you to attend an outpatient clinic instead.
During your first appointment, we will talk with you about any difficulties you may be having in your everyday life. This may include looking after yourself, managing at home, work or education, and taking part in hobbies or activities you enjoy.
This conversation helps us understand your individual needs and how your health condition affects your daily life. We can then work with you to identify ways to support you, improve your independence, and help you do the activities that are important to you.
Your first appointment usually lasts around one hour.
